Some of the biggest names in US industry have been hit by unprecedented crisis. Names like-GM, Ford, Chrysler, Citibank, not to speak of Lehman Bros. and AIG- have turned out to be in incredibly bad shape.

Who is accountable? Is it the top management team responsible for strategy? Is it an all round failure, which was largely brought about by complacency?
